After clicking the links to the printable Application and Notice to Vacate forms below (The Maintenance Request is submittable online),
1) Adjust your browser's printer options (located under "File" at top left of the window in which you will view the form. If not, adjust the setup options for your specific printer.) to NOT print a header or footer (URL links and/or page numbers, etc.)
2) Print, using only black ink on legal size (8" X 14"), white paper.
3) Fill in all applicable blanks.
4) Sign the form.*
5) Mail or hand deliver the completed form to FitzGerald Properties:
6) Return to the previous page from the form page by clicking your browser's back button.
